Chronic Kidney Disease (CKD) Management

CKD often develops without symptoms in the early stages. That is why routine blood tests (creatinine, GFR) and urine analysis matter, especially if you have diabetes or high blood pressure. Dr. Gupta evaluates each patient's kidney function, determines the CKD stage using KDIGO guidelines, and builds a treatment plan around dietary changes, blood pressure control, and medication adjustment. For patients with diabetic or cardiorenal CKD, newer therapies like SGLT2 inhibitors and finerenone are used where appropriate.

When kidney function declines to the point where dialysis or transplant may be needed, Dr. Gupta coordinates that planning early so patients and families have time to prepare.

Glomerular Diseases

Glomerular diseases affect the tiny filtering units inside the kidneys. They can cause protein or blood in the urine, swelling, and rising blood pressure. Dr. Gupta has published research on complement 3 glomerulopathy (C3G), a rare glomerular condition, and presented findings at the World Congress of Nephrology.

Conditions treated include glomerulonephritis, nephrotic syndrome, IgA nephropathy, lupus nephritis, and MGRS (monoclonal gammopathy of renal significance). Treatment typically involves kidney biopsy interpretation, immunosuppressive therapy, and long-term follow-up.

Dialysis Services

When the kidneys can no longer filter waste effectively, dialysis keeps things running. Dr. Gupta oversees haemodialysis care, including vascular access planning (AV fistula, graft, or catheter), adequacy testing, and managing common complications like anaemia, mineral bone disorder, and fluid overload. Peritoneal dialysis is also an option for patients who prefer home-based treatment.

Dr. Gupta's ongoing research includes work on non-renal causes of anaemia in CKD (under review at JAPI) and L-carnitine supplementation for dialysis patients with EPO-resistant anaemia.

Kidney Transplant Evaluation & Follow-Up

For patients with end-stage kidney disease, a transplant often offers the best quality of life. Dr. Gupta performs detailed pre-transplant evaluation for both donors and recipients, coordinates the required tests, and manages post-transplant care including immunosuppression and complication screening.

This expertise comes from training at SGPGIMS Lucknow, which runs one of India's most active deceased donor transplant programmes. Dr. Gupta chaired the session on living kidney donor evaluation at the 2025 Genetics and Kidney Diseases conference.

Hypertension & Cardiorenal Syndrome

High blood pressure and kidney disease feed off each other. Each can cause or worsen the other, and many patients deal with both simultaneously. Dr. Gupta holds postgraduate diplomas in Hypertension Management and Advanced Cardiology from the Royal College of Physicians, London. This dual training is particularly useful for managing secondary hypertension, resistant hypertension, and cardiorenal syndrome, where the goal is protecting both heart and kidney function at the same time.

Acute Kidney Injury (AKI)

Acute kidney injury is a sudden drop in kidney function, often triggered by dehydration, infection, medication side effects, or urinary obstruction. Quick evaluation is important because many causes are reversible if caught early. Dr. Gupta identifies the underlying cause, manages recovery, and works to reduce the risk of it progressing to chronic kidney disease.

Point-of-Care Ultrasound (POCUS) in Nephrology

Bedside ultrasound lets Dr. Gupta assess kidney size, structure, fluid status, and vascular access without waiting for a formal imaging appointment. It means faster clinical decisions and more tailored treatment. Dr. Gupta was an invited speaker on "Role of POCUS in Nephrology" at the Fourth National Update on Nephrology and Kidney Transplant (2025).

Electrolyte & Acid-Base Disorders

Imbalances in sodium, potassium, calcium, or phosphorus can cause anything from fatigue and muscle cramps to serious cardiac complications. Dr. Gupta diagnoses the underlying cause and develops a targeted plan to restore balance safely.
